When submitting your field service request form, please provide as many details as possible so that we can complete the necessary repairs in a single visit to your location. In order to effectively prepare our service technician, we require 2-3 pictures that show the affected area.

The pictures should help explain:
• Where on the tank the leak is (i.e. Front, rear, side, notch, ect)
• Is the repair accessible in the truck. Picture from 3-4 feet away will give us an idea of access ability.
• A close-up of the leak. This will help us to determine the significance of the leak You do not need to send pictures of:

• water on the ground
• multiple close-up pictures
• Blurry pictures

If you are unable to send pictures, please send an email with subject line "Unable to send pictures" to [email protected] for further instructions.

Chose Files: (Still Photos – Give Context) Files accepted pdf, jpeg, jpg, png. No more than 5 files accepted and each file can not exceed 4 meg
